What is TMJ disorder and what causes it?

The temporomandibular joint (TMJ) is the hinge joint connecting your lower jaw to your skull, located just in front of each ear. TMJ disorder (TMD) involves pain, clicking, and dysfunction in this joint and the surrounding muscles — affecting an estimated 10 million Americans. It is one of the more underdiagnosed and undertreated conditions in musculoskeletal health.

Common contributing factors include teeth grinding (bruxism), bite misalignment, jaw trauma, stress-related clenching, arthritic changes in the joint, and — critically — upper cervical spine misalignment. The atlas vertebra influences the position of the head relative to the jaw, and when it shifts out of alignment, the mechanical loading of the TMJ can become asymmetric, accelerating wear and contributing to pain.

Why TMJ treatment is sometimes incomplete

Dental treatment for TMJ — night guards, splints, orthodontics, and in more involved cases surgery — addresses the oral mechanics of the condition. These are appropriate and valuable interventions. However, they cannot address upper cervical misalignment that may be altering jaw mechanics in the first place.
